I have been worshiping at Evangelical Lutheran Church of Cokato, MN and/or another Christian church on a regular basis for several months (including during the months immediately preceding this request for membership) and now want to become a confirmed, voting member of Evangelical Lutheran Church of Cokato, Minnesota.
*restoration to confirmed membership (having already been a confirmed member of this or another Lutheran church)
**reception into confirmed membership by Affirmation of Faith, and baptism if not already baptized, after regularly attending worship at a Lutheran Church over a period of several months
***reception into confirmed membership by Affirmation of Faith, and baptism if not already baptized, after completing a period of instruction in the Christian faith as confessed in the teachings of the Lutheran Church--speak with the pastor if you choose this option to work out details.
****reception into baptized (non-confirmed) membership. Note: This category is for children and youth under confirmation age or for those who cannot make any affirmation of faith due to severe disability. Children previously baptized in the name of the Triune God are received with parents/guardians. Children and younger youth not yet baptized are received through Holy Baptism.
I understand that as a confirmed voting member of Evangelical Lutheran Church of Cokato, I will be expected to make a public affirmation of faith and -
A) make regular use of the means of grace, both Word and sacraments and
B) live a Christian life in accordance with the Word of God and the teachings of the Lutheran church and
C) support the work of this congregation, synod, and the churchwide organization of the Evangelical Lutheran Church in America through contributions of time, abilities, and financial support as a biblical steward (from the constitution of the congregation, C8.04).
